Why Do My Legs Hurt to Touch? It Could Be More Than Muscle Pain

Have you ever noticed that your legs feel sore or painful when someone presses on them—even though you haven’t exercised or injured yourself recently? While occasional muscle soreness is common, persistent tenderness isn’t something you should ignore. Understanding the cause is the first step toward finding the right treatment.

Common Causes of Leg Tenderness

Painful legs can result from a variety of conditions, including:

  • Muscle soreness or overuse after exercise or physical activity
  • Bruising or a recent injury
  • Varicose veins or poor circulation
  • Lymphedema, where fluid builds up due to a compromised lymphatic system
  • Lipedema, a chronic condition that causes painful fat accumulation, most commonly in women

While some causes are temporary, others require a thorough assessment to identify the most appropriate management.

Could It Be Lipedema?

Lipedema is often misunderstood and frequently mistaken for obesity or simple weight gain. It primarily affects women and commonly develops or progresses during hormonal changes such as puberty, pregnancy, or menopause.

Common signs include:

  • Legs that are painful or tender to touch
  • Easy bruising
  • A feeling of heaviness in the legs
  • Symmetrical enlargement of both legs
  • Difficulty reducing leg size despite healthy eating and exercise
  • Feet that are usually not affected

Because these symptoms develop gradually, many people live with lipedema for years before receiving an accurate diagnosis.

A Patient’s Story

A 45-year-old office worker came to our clinic after falling onto her right hip. She was diagnosed with a mild hip strain and responded well to physiotherapy. Her hip pain improved, and she regained her strength and mobility.

However, she continued to experience aching, heaviness, and tenderness throughout her legs, particularly after sitting for long periods at work.

During a comprehensive assessment, it became clear that her persistent leg pain was not coming from the healed hip injury. Instead, her symptoms were consistent with lipedema, a condition that had likely been present for years. The fall did not cause lipedema, but it may have temporarily increased inflammation and made her existing symptoms more noticeable.

With education, an individualized exercise program, manual therapy, and strategies to help manage swelling and discomfort, she reported improved walking tolerance and reduced leg pain.

How Can Physiotherapy and Massage therapy Help?

A physiotherapy assessment can help determine whether your symptoms are related to a muscle injury, circulation, the lymphatic system, or another condition such as lipedema.

Treatment may include:

  • Education about your condition
  • Individualized strengthening and mobility exercises
  • Manual therapy, when appropriate
  • Manual Lymphatic Drainage (MLD) for suitable patients
  • Guidance on compression therapy and activity modification

The goal is to improve comfort, mobility, and your ability to participate in everyday activities.

When Should You Seek an Assessment?

Consider seeing a physiotherapist or massage therapist if you have:

  • Leg pain that persists for several weeks
  • Legs that are consistently tender to touch
  • Frequent bruising without significant injury
  • Heavy or swollen legs that worsen throughout the day
  • Symptoms that interfere with walking, work, or daily activities

Take-Home Message

Not all leg pain is caused by overuse or injury. If your legs are consistently tender to touch, bruise easily, or feel heavy, there may be an underlying condition contributing to your symptoms. A thorough assessment can help identify the cause and guide an appropriate treatment plan.

At Vaughan Wellness Centre, we provide comprehensive physiotherapy assessments for leg pain, lymphedema, and lipedema. If you’re experiencing persistent symptoms, we’re here to help you better understand your condition and develop a personalized management plan.
